Modern Principles of Management and Treatment in Child and Adolescent Psychoses – the Pharmacogenetic Testing

Introduction: We approached an integrated research-action model of different intervention strategies and pharmacogenetic testing in child and adolescent psychosis and for UHR – ultra high risk children for psychosis. Methods: Our research was coducted in the period 2007-2015 on children and adolescents with psychosis - a group of 85 children-G1, who benefited in choosing the pharmacotherapy from pharmacogenetic testing and 90 without testing-G2 and 105 UHR children for psychosis -50 benefited of pharmacotherapy after pharmacogenetic testing/extensive psychosocial interventions and 55 received interventions without testing. The efficacy of the chosen therapy in correlation with the pharmacogenetic testing was evaluated through the mean change in the PANSS total scores, in the CGI-S/I, CGAS, CD-RISC (Connor-Davidson Resilience Scale), UKU from baseline till endpoint. Our main focus was to investigate wich types of intervention strategies would be most indicated, in order to prevent or postpone the transition to psychosis. The CYP2D6*3*4*5*41 allelic varients genotypes were determined by measuring allele-specific fluorescence using the software for allelic discrimination. Results: Results, show statistically significant differences of the clinical scores between the studied groups: for those subjects who benefited of pharmacogenetic testing, the PANSS, the resilience scores, the global functioning scores prove a higher clinical improvement, a better compliance and lower UKU side effects scores. Conclusions: Our research was a proof, sustaining the use of pharmacogenetic and testing in clinical practice, in the frame of crisis intervention for a personalized, tailored therapy in child and adolescent psychosis, as a fruitful pathway of intervention and care.
